See how to combine four lid blepharoplasty with rhinoplasty to create a more rested appearance and reduce a bulbous tip and dorsal hump.


Lately we've been discussing the many facets of nasal bridge hump removal. It is pretty easy to see how straightening the profile line by removing a nasal bump can improve the appearance of the nose. In the first part of our case study I'd like to show you how we can combine an eyelid tuck with rhinoplasty to create an even more dramatic change in appearance.
 

Remove eyelid bags and excess lid skin

This patient had several concerns. The first was bags that were evident in her lower eyelids that made her look tired. The second problem was the fullness to her upper eyelids that made it difficult to wear upper eyelid makeup.


 


How to use a hidden lower lid blepharoplasty incision

To address the patient's problems I first conservatively removed some excess upper eyelid skin to help improve her upper lid show. Next, I performed a lower eyelid blepharoplasty using a hidden incision placed on the inner aspect of the lower eyelid. This transconjunctival incision is a great way to access the area without making a visible scar. The lower lid blepharoplasty involved removal of a small amount of excess eyelid fat. No lower lid eyelid skin needed to be removed given the patient's relatively young age and good skin eleasticity.



You can see how surgery was able to improve the shadowing in the patient's lower lid area creating a smoother contour. Also, her upper eyelids have a naturally more open appearance. The patient is also more easily able to wear makeup on her upper eyelids. Importantly, I didn't over resect the upper eyelid, but rather maintained the patient's youthful per-orbital fullness.
